PRE-OPENING INSPECTION. THE FOLLOWING ITEMS WERE DISCUSSED:
-ONLY ONE CFPM CERTIFICATION IS REQUIRED. AT LEAST TWO ARE RECOMMENDED. -PROVIDE COPIES OF ALL FOOD HANDLER CERTIFICATIONS AT ORIGINAL INSPECTION IN 30 DAYS. -THE FACILITY HAS A BODILY FLUIDS CLEAN UP KIT. -HAVE ALL EMPLOYEES SIGN 1B FORMS AND KEEP ON SITE. -FOOD THERMOMETER IS AVAILABLE. -CHLORINE SANITIZER AND TEST STRIPS ARE AVAILABLE. -DRAIN PIPES MUST TERMINATE 1" ABOVE THE FLOOD RIM OF THE DRAIN. -THE TABLE FOR THE DOUGH MIXER HAS NOT YET BEEN DELIVERED. A TEMPORARY RISER IS IN PLACE AND CAN BE USED UNTIL THE TABLE ARRIVES. -REFERRAL FORM HAS BEEN SIGNED BY ALL INSPECTORS. -THE FOOD LICENSE APPLICATION AND $175 FEE HAS BEEN RECEIVED. THIS ESTABLISHMENT IS PERMITTED TO OPERATE AS A MEDIUM RISK FACILITY PER THE AUTHORITY OF THE KANKAKEE COUNTY HEALTH DEPARTMENT. AN ORIGINAL INSPECTION WILL OCCUR IN ABOUT 30 DAYS. |
